Tirzepatide vs. Semaglutide

How they work

Both slow gastric emptying, cue satiety in the brain, and blunt the "food noise" that makes constant snacking feel automatic. Tirzepatide adds GIP activity, which appears to enhance insulin response and, based on trial data, contributes to greater average weight loss.

Weight loss outcomes

The STEP trials of semaglutide (2.4 mg) showed roughly 15% average body-weight loss at 68 weeks. SURMOUNT-1, the pivotal trial of tirzepatide (15 mg), showed roughly 21% at 72 weeks. Real-world results vary widely — your ramp-up, adherence, sleep, protein intake, and activity all matter.

Side effects

The side effect profile is broadly similar — nausea, constipation, reflux, fatigue, occasional injection-site reactions. Some patients tolerate tirzepatide better despite the higher potency; others find semaglutide easier. There is no reliable way to predict this in advance. Both carry a boxed warning about thyroid C-cell tumors based on rodent studies.

Cost and access
